Apple doesn't allow most Bluetooth connections, they require it to use an Apple chip and be Apple approved so they can get money from the Bluetooth accessories.

The only Apple approved Bluetooth devices I know of are the GoPoint BT1 and the PLX Kiwi 3. If you want to use a cheap ELM from Amazon/eBay you need to get one that is WiFi and not Bluetooth.

So how do I get a refund for the app purchase

You have to go through Apple to get a refund.

Recently bought a Bluetooth ELM327 device that claims to be iPhone compatible.. having trouble with it so in light of this post I'm thinking I've been taken for a ride and the eBay seller might be dodgy.
